About The Position

As the Clinical Care Provider for the Hope Hospice Center, Partners In Home Care is dedicated to delivering compassionate, expert care that upholds the dignity, comfort, and unique needs of every patient and their loved ones. The Flex Shift CNA position is designed to provide additional staffing flexibility based on patient census, acuity, and operational needs of the Hospice Center. The position includes a regularly scheduled bid of a minimum of 24 hours per week or .6 FTE and an additional 12 hours per week or .9 FTE of flex shift availability. Flex shift positions are established based on the staffing needs of the Hospice Center and are intended to assist the organization in maintaining appropriate staffing while responding to fluctuations in patient census and care needs. Your role as a CNA is not just a job but a heartfelt opportunity to provide comfort and support to those nearing the end of life. Our CNA’s provide personal healthcare and related services to patients in the Hope Hospice Center. This position is vital in creating a safe and clean environment, working cooperatively with patients and their families. Our CNAs are key members of each patient’s care team, committed to making a meaningful difference daily.

Requirements

  • High school diploma or equivalent
  • Current CNA certification
  • Valid driver’s license, reliable personal vehicle, and appropriate auto insurance
  • Ability to work independently, communicate effectively, and follow a plan of care
  • Commitment to compassionate, patient-centered service
  • A Flex Shift CNA must maintain a regularly scheduled bid of at least 24 hours per week and provide an additional 12 hours per week of flex shift availability.
  • Maintain a regularly scheduled bid of at least 24 hours per week and accept an additional 12 hours per week flex shift obligation; or
  • If regularly scheduled for more than 24 hours per week, be willing to have scheduled hours reduced to a minimum of 24 hours per week based on Center staffing needs and census.

Responsibilities

  • Provide personal care in various settings the patient prefers, including homes, nursing homes, assisted living facilities, eldercare facilities or community partner’s inpatient care settings.
  • Act as the “eyes and ears” for the broader hospice team, observing the patient’s condition and reporting any changes or concerns.
  • Work alongside passionate healthcare professionals, providing a supportive and compassionate environment for patients and their families.
  • Help with the patient’s personal hygiene needs, including bathing, dressing, grooming, and oral/mouth care.
  • Perform light housekeeping tasks related to the patient, such as making the patient’s bed and tidying the patient’s immediate environment.
  • Be flexible and adaptable, ready to adjust to changing patient needs and care requirements.
