About The Position

Certified Nurse Practitioner – Knox County Community Health Center (KCCHC) Location: Knox County, Ohio, USA Job Type: Full-Time Reports to: Chief Nursing Officer Overview: Knox County Community Health Center (KCCHC) is seeking an experienced Certified Nurse Practitioner to join our integrated healthcare organization, which provides medical, dental, and behavioral health services to the community. KCCHC is located in semi-rural Knox County, offering the unique opportunity to address healthcare disparities in a designated health professional shortage area, while being close to larger metropolitan centers. Our mission is to improve the overall health of the community by providing accessible, high-quality care to underserved populations, particularly in areas with limited access to primary medical, dental, and behavioral health services. As a Public Entity, Co-Applicant Federally Qualified Health Center (FQHC) model, this position will also support the mission and vision of the health department. As a Certified Nurse Practitioner at KCCHC, you will play a vital role in providing high-quality patient care while contributing to the organization's strategic direction. You will collaborate closely with the healthcare team to enhance access to care, improve patient outcomes, and support the clinic's financial sustainability.

Requirements

  • Master’s or Doctorate degree in Nursing from an accredited program.
  • Active and unrestricted Nurse Practitioner (NP) license in the state of Ohio.
  • National board certification as a Nurse Practitioner (e.g., AANP, ANCC) in a relevant specialty.
  • Minimum of two (2) years of clinical experience as a Nurse Practitioner, preferably in primary care, community health, or a similar setting preferred
  • Current DEA registration for prescribing controlled substances (if applicable).
  • Willingness to participate in community outreach and health education initiatives.

Responsibilities

  • Accurately and thoroughly document each patient encounter, including services provided and relevant observations, in a clear, legible, and timely manner.
  • Ensure the completion and accuracy of all medical record entries, adhering to documentation standards and guidelines.
  • Submit documentation with appropriate coding for each visit to support billing and compliance requirements.
  • Assess patient health risks, behaviors, and contributing factors to develop appropriate care plans.
  • Provide counseling and education to patients, families, and caregivers regarding health conditions, treatment options, and preventive care.
  • Diagnose and manage common health conditions, symptoms, and diseases within the nurse practitioner's scope of practice.
  • Oversee outpatient care following medical staff-approved protocols and clinical guidelines.
  • Educate and counsel patients on health status, diagnostic test results, disease processes, treatment plans, and discharge instructions.
  • Refer patients to physicians or other healthcare specialists for conditions beyond the NP’s scope of practice.
  • Obtain and document comprehensive medical and social histories, performing physical examinations, including rectal and pelvic exams when indicated.
  • Order and interpret diagnostic tests and therapeutic interventions, such as laboratory tests, imaging studies, medications, treatments, and IV therapies.
  • Conduct consultations and accurately document findings and recommendations.
  • Provide care to both critical and non-critical patients, adhering to scope of practice and clinical competencies.
