About The Position

Traditional Home Health Care is seeking a Certified Nurse Aide (CNA) to join our team and make a difference in someone's life. Our CNAs provide care to patients in their homes, covering Carbondale, PA, and surrounding areas. We strive to schedule visits within a sensible geographic territory daily. Requirements

• Current Pennsylvania certification as a Certified Nurse Aide
• Must have a current PA driver’s license
• Ability to travel to multiple patient assignments daily
• Ability to work independently with minimal supervision
• Excellent organizational and time management skills
• Strong interpersonal skills
• Strong verbal and written communications skills

Responsibilities

• Administers personal care and ADL assistance to patients according to the individual's care plan.
• Reports patient status changes to nursing supervisor on a continuous basis.
• Prepares, maintains, and submits documentation as required in a thorough and timely manner.
