The PACE Certified Nurse Assistants (CNA) are responsible for providing the highest quality of skilled and non-skilled care in the day center, or in the community where the participants live. The CNA will follow the established plan-of-care for each participant that may include ADL activities, meal preparation, monitoring vital signs, and communicating any concerns or changes to the proper IDT member. PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.
