About The Position

Provides hands on personal care and activities of daily living according to the aide care plan as directed by the Registered Nurse or Therapist. Requirements

  • High School Diploma/GED
  • 1 year Experience in a healthcare setting as a nursing assistant or nursing aide.
  • Builds strong relationships with patients, family members, caregivers, and members of the interdisciplinary care team.
  • Knowledge of medical terminology.
  • Strong time management skills.
  • Ability to drive to patients homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs.
  • Ability to work in patient homes.
  • Ability to work with and support terminally ill patients and their family members/caregivers.
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • CNA - Certified Nursing Assistant - State of Pennsylvania Upon Hire

Nice To Haves

  • Home Health/Hospice - must possess and maintain personal car insurance at PA State minimum requirements.

Responsibilities

  • Provides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care.
  • Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate.
  • Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager as appropriate.
