About The Position

Lehigh Valley Health Network (LVHN) is seeking a Certified Nurse Assistant (CNA) for their Inpatient Hospice Unit, working full-time evening shifts. This role involves providing hands-on personal care and assisting with daily living activities according to the patient's care plan, as directed by a Registered Nurse or Therapist. The CNA will also provide other support services as a member of the care team, ensuring compassionate and high-quality patient care within a nationally recognized health network known for its commitment to teamwork, compassion, and technology.

Requirements

High School Diploma/GED
1 year Experience in a healthcare setting as a nursing assistant or nursing aide.
Builds strong relationships with patients, family members, caregivers, and members of the interdisciplinary care team.
Knowledge of medical terminology.
Strong time management skills.
Ability to drive to patients homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs.
Ability to work in patient homes.
Ability to work with and support terminally ill patients and their family members/caregivers.

Home Health/Hospice - must possess and maintain personal car insurance at PA State minimum requirements.

Responsibilities

Provides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care.
Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate.
Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager as appropriate.
