About The Position

A Certified Nurses Aide is a primary member of the nursing team who provides quality care to residents in compliance with philosophies, objectives, resident's rights and established policies and procedures. This role actively participates as a team member showing respect, dignity, accountability, professionalism and ethical values toward residents, family and staff while maintaining confidentiality. The aide follows hierarchy and performs duties within the standards of practice, meeting resident ADL needs including personal hygiene, oral/denture care, bathing, grooming, and dressing. They follow and comprehend the resident plan of care regarding physical and emotional well-being, including mobility, hygiene, and emotional care. The aide must have a working knowledge of Resident Rights, abuse and neglect policy, including report requirements and protocol, and follow all policies regarding safety measures and equipment competency. They consistently follow Infection Control policies and measures regarding hand washing, standard precautions, linens, and maintaining a clean environment. The aide meets elimination needs of residents per policies, documents toileting times, and follows routine bowel regimes. They follow Rehab Nursing measures, encouraging independence, range of motion, ambulating, positioning, alignment, transfers, and transport. The aide pays special attention to good body mechanics and safe lifting techniques, and is responsible for checking the activity calendar and ensuring residents are transferred to and from activities in a timely manner. They assist with admissions, discharges, transfers, and transport to appointments. The aide reports any resident needs, signs or symptoms of illness or distress, injuries, or accidents to the Nurse on duty. They measure, record, and document vital signs, height/weight, I&O's, and any pertinent information to the resident care plan, reporting out-of-limit findings to the Nurse. The aide participates in resident care conferences and the care planning process. They follow personnel policies regarding attendance, breaks, meals, staff meetings, mandated requirements, float assignments, and dress code. Work schedules may change based on department needs. The aide has a working knowledge of Corporate Compliance regulations and reports potential violations. They are familiar with and participate in Continuous Quality Improvement principles, focusing on customer service needs. The aide must possess the highest ethical standards with respect to discretion and confidential information and provides age-appropriate care to adult and geriatric patients/residents. They are responsible for attending all mandatory educational programs and maintaining required CEUs annually. Individuals may be required to work hours beyond those normally scheduled, and work schedules may be subject to change. Requirements

  • Certified as a Nurse Aide with the New York State Department of Health.
  • Current Heartsaver certification required.

  • Nursing Assistant experience preferred.

Responsibilities

  • Meets resident ADL needs including personal hygiene, oral/denture care, bathing, grooming, and dressing as per policies.
  • Follows and comprehends resident plan of care regarding physical and emotional well-being of each resident including but not limited to all mobility, hygiene needs, and emotional care as directed by individual plan of care.
  • Has a working knowledge and demonstrates usage of Resident Rights, abuse and neglect policy, including report requirements and protocol.
  • Follows all policies regarding safety measures and equipment competency, including supply needs and replenishment.
  • Consistently follows all Infection Control policies and measures regarding hand washing standard precautions, linens, clean and orderly environment including all resident care areas.
  • Meets elimination needs of all residents per policies and documents toileting times and initials incontinence and continence. Follows routine bowel regimes, documents and reports to Nurse.
  • Follows Rehab Nursing measures. Some examples are encouraging independence and decision making, range of motion, ambulating, positioning and alignment, transfers and transport, moving resident up in bed, etc.) Pays special attention to good body mechanics and safe lifting techniques.
  • Is responsible to look at the activity calendar at the beginning of each shift and assures that all residents are transferred to and from all activity programs in a consistent, timely manner.
  • Assists with admissions, discharges, transfers, and transport to appointments.
  • Reports any needs of resident, signs or symptoms of illness or distress, injuries or accidents including falls, skin tears, bruises, and red areas to Nurse on duty.
  • Measures, records and documents vital signs, height/weight, I&O's and any information that is pertinent to resident care plan, resident care profile and reports all out of limit findings to the Nurse on duty.
  • Participates in resident care conferences as appropriate and the process of care planning in a positive and constructive manner.
  • Follows all pertinent personnel policies re: attendance, breaks, meals, staff meetings, mandated requirements per policy, float assignments, dress code etc.
  • Has a working knowledge of Corporate Compliance regulations and promptly reports any potential Corporate Compliance violations to the Corporate Compliance Officer.
  • Is familiar with and demonstrates Continuous Quality Improvement principles and participates in the process as appropriate, focusing on meeting the customer service needs of the internal and external customer.
  • Provides age appropriate care to adult and geriatric patients/residents.
  • Is responsible for attending all mandatory educational programs as required by position including successful completion of orientation program and maintains 12 CEU's per year. (6 CEU's per 6 months)
