About The Position

Elara Caring is seeking a compassionate Certified Nurse Aide to provide home health care under the direction of a Registered Nurse. The role provides hands-on personal care and support with daily activities, mobility, and safety while following each patient’s individualized care plan. The aide monitors and reports changes in patient condition, documents care accurately, protects patient privacy, and participates in required training. The position requires travel between patient homes and assigned work locations.

Requirements

• Current state Home Health Aide certificate or Certified Nursing Aide certification.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation and protect patient privacy.
• Participate in required training.
• Travel between patient homes and assigned work locations.
