About The Position

This position, under the supervision and direction of a licensed nurse, is responsible for preparing, administering, and recording specific oral, nasal, transdermal, rectal, inhalant and ophthalmic scheduled prn medications to the residents of Lakeland Health Care Center (LHCC). The role involves maintaining in-depth knowledge of medication actions, administering and documenting medications, recognizing and communicating changes in residents’ conditions, taking and recording vital signs, providing personal hygiene, assisting with resident mobility and transfers, serving meals, cleaning food preparation areas, monitoring food supplies, using assistive equipment, documenting cares, maintaining resident rooms, reviewing CNA Summary Cards, assisting with plans of care, using standard precautions and isolation techniques, performing simple treatments, participating in inservice education, delivering care in compliance with the Resident Bill of Rights, assisting with orientation of new CNAs and CNA student clinical experience, communicating concerns, responding to emergencies, assisting with post mortem care, assisting with resident admissions, labeling personal items, participating in Quality Improvement activities, and assisting residents with medical appointments and transportation.

Requirements

  • High school diploma or equivalent competencies
  • Two to three years of relevant prior experience
  • Knowledge of medical terminology
  • Basic arithmetic skills
  • Ability to perform manual tasks associated with personal care of elderly
  • Knowledge and understanding of Resident Bill of Rights
  • Ability to work independently and as part of a team
  • Good oral and written communication skills
  • Wisconsin Nursing Assistant Certification, and on State Registry for Nursing Attendants
  • State of Wisconsin Medication Administration Certification
  • Criminal background check free of convictions of abuse of individuals or misappropriation of property while in a formal caretaking setting
  • All staff of Lakeland Health Care Center may be called upon to respond to emergency situations assisting residents in their physical positioning.
  • All employees may be required to work overtime as determined by the department head.

Responsibilities

  • Maintain in-depth knowledge of medication actions, medication effects on body systems and side effects from medication in relation to geriatric residents.
  • Administer and document scheduled medications prescribed for routine administration according to established LHCC medication administration policies and procedures.
  • Understand and effectively carry out verbal and/or written instructions.
  • Recognize changes in residents’ conditions and effectively communicate those changes to the Unit RN or LPN.
  • Take and record resident’s blood pressure and pulse as scheduled and in conjunction with prescribed medication therapy and subsequently report any significant changes compared to the prior reading to the Unit RN or LPN.
  • Report changes in resident's physical and/or mental status to the Unit RN or LPN immediately.
  • Provide personal hygiene including oral care, bathing, hair shampooing, shaving, skin care, nail care, and hand washing as well as assisting with toileting including bowel and bladder retraining and perineal care.
  • Assist/perform resident mobility transfers, ambulation, position changes and range of motion.
  • Serve resident meals and assist resident during meals.
  • Interpret modified diet menus and prepare accordingly.
  • Clean and sanitize food preparation utensils, dishes, equipment and resident serving/dining areas.
  • Monitor refrigerators for proper labeling, dating, storing, rotating and use of foods.
  • Order food supplies from the main kitchen.
  • Employ appropriate assistive equipment such as gait belts, mechanical lifts and correct body mechanics when lifting, positioning and/or transferring residents and/or their belongings.
  • Document cares given such as temperature, pulse, respiration, weight, intake/output and ambulation on flow sheets, target behavior forms, nutritional intake sheets, and required electronic charting.
  • Assist residents in maintaining the cleanliness and neatness of resident's rooms including making beds and disposing of soiled linens and clothing.
  • Review Certified Nursing Assistant (CNA) Summary Cards for assigned residents daily and discuss suggested changes with Nurse Manager as needed as well as assist with pre-staffing information collection.
  • Help to establish/update plans of care (Kardex) for residents by communicating with the Unit Manager and members of the interdisciplinary team.
  • Use standard precautions for infection control as well as using isolation techniques, including the use of a particulate mask.
  • Perform simple treatments as assigned by the Unit RN or LPN within the limits of ability, training and guidelines of the Wisconsin Administrative Code Chapter HFS129.
  • Participate in at least twelve (12) hours each year of inservice education programs to increase knowledge and skills as requested to maintain certification on the Wisconsin State Registry as well as attend mandatory in-services.
  • Obtain four (4) hours of Certified Medication Assistant (CMA) in-service instruction per year.
  • Deliver care in compliance with the Resident Bill of Rights by maintaining confidential resident care information and respecting residents’ rights and privacy.
  • Assist with orientation of new CNAs on the unit as necessary.
  • Assist with CNA student clinical experience.
  • Communicate problems or concerns with team members to assure issues affecting resident care outcomes or general operations of the facility are handled efficiently.
  • Respond to emergency situations per LHCC Emergency Response Plan.
  • Respond to resident emergencies such as bleeding, falls, seizures and choking per policies and procedures.
  • Assist with post mortem care.
  • Assist with the admission of new residents.
  • Label residents’ personal items.
  • Participate in Quality Improvement activities.
  • Assist residents to get ready for medical appointments.
  • Assist residents to safely board the transportation vans.
  • Monitor positioning and safety of residents during transport.
  • Assist residents at the appointment site.
  • Upon return to the facility, assist resident back to their room and help with ADL’s, as needed.
  • Demonstrate a commitment to county safety and risk management efforts.
