About The Position

Annandale Village is a one-of-a-kind nonprofit community in Suwanee, GA, where we proudly serve adults with developmental disability and acquired brain injuries. We are a true community where our residents, known as Villagers, are supported in living with dignity, purpose, and independence. We’re the only nonprofit in the southeast offering a full continuum of care for the people we serve. Our 55-acre campus includes: Two assisted living buildings serving 33 Villagers, Eight Personal Care Homes (independent, semi-independent, and cottages) serving up to 100 Villagers, A 32-bed skilled nursing facility—the only one in Georgia dedicated solely to adults with developmental disabilities, A Medicaid waiver day program with approximately 33 participants, A community-based independent living program, All In, currently serving 26 individuals. Requirements

  • Must be a Certified Medication Aide (CMA) in good standing and meet all applicable federal and Georgia state certification requirements.
  • Speak and understand English and able to read, write and follow oral and written instructions.
  • Must have a High School Diploma or general education diploma (GED).
  • Must be 18 year of age to operate Annandale Vehicles/Golf Carts, employee must have a valid Class C Georgia Driver’s License with 3 years driving experience, proof of vehicle insurance, and meet company policy regarding MVR requirements.
  • Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
  • Must undergo and pass a drug screening and screening for Tuberculosis.
  • Ability to apply common sense understanding to carry out detailed written or oral instructions.
  • Ability to deal with problems involving few concrete variables in standardized situations.
  • While performing the duties of this job, the employee is regularly required to use hands; use fingers; handle or feel; reach with hands and arms; talk; and hear.
  • The employee frequently is required to sit, stand, and walk; climb or balance; stoop, kneel, crouch, bend, or crawl.
  • Employee is required to transport (drive) and assist Villagers in and out of vehicles.
  • Employee is required to perform CPR/First Aid.

Responsibilities

  • Performs non-professional direct care duties following the care plan under the direction and supervision of the Administrator /LPN Nurse Manager.
  • Provides ADL support.
  • Maintains a positive physical, social, and psychological environment for the Villagers at the Assisted Living Center.
  • Assists Villagers with activities of daily living such as bathing, dressing, grooming, escorting, medication supervision and other functions of designated in the Villager’s Service Plan.
  • Provide functions as directed by the Nurse Manager including the taking and recording of temperature, pulse and respiration and monthly weights and blood pressures.
  • Observe and report immediately information in the chart using approved charting criteria.
  • Report immediately to the Nurse Manager, Administrator and or family all accidents, or incidents, record all necessary information in appropriate system and render first aid.
  • Assist as directed in proper admission, transfer, and discharge of Villagers.
  • Monitors Villagers medication related supplies and medication quantities.
  • Watches for and recognizes possible medication side effects, adverse reactions and allergic reactions and notifies LPN Nurse Manager should any occur.
  • Submits daily shift reports electronically to Nurse Manager and Administrator.
  • Follows proper medication storage and disposal procedures.
  • Accurately documents medication provided or refused by the Villager in the MAR according to policy and professional standards.
  • Train new staff on current procedures and protocols.
  • Assists Villagers with activities of daily living care.
  • Provides non-professional direct care including perineal care, catheter care, and assistance w/ bed mobility, positioning in / out of wheelchairs / chairs, bed, showers, bathtubs, etc.
  • Completes ADL and service tracking documentation daily.
  • Assists Villagers with preparation for medical tests and / or exams.
  • Transport Villagers to medical/hospital visits outside of facility as assigned.
  • Assists in activities program for Villagers as assigned.
  • Completes clinical and behavioral documentation accurately and timely.
  • Reports changes in Villager’s condition and completes incident reports accurately and timely.
  • Completes internal documentation / requests for any repairs needed to equipment, furnishings / buildings / vehicles as appropriate.
  • Understands and uses Universal Precautions and follows established infection control, hazardous communication, and other safety rules.
  • Ensures cleanliness of assigned Villagers’ rooms; and promptly reports all violations of safety and sanitation rules to the Administrator.
  • Performs laundry including washing, drying, and ironing.
  • Informs the Administrator of needs in assigned areas.
  • Participates in the Quality Assurance initiatives of the community.
  • Protects the rights, dignity and individuality of Villagers and others.
  • Complies with all Annandale policies, procedures, and dress code; is punctual and reliable in performance of his/her duties.
  • Follow established safety procedures when performing tasks and/or working with equipment and / or Villager.
  • Participates in mandatory in-service training, staff meetings and educational activities.
  • Continues to maintain in improving job related skill sets and departmental services.
  • Meets all state licensing and contract requirements and abides by Annandale policies and procedures.
  • Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the facility residents.
