About The Position

The Certified Medical Coder maintains and organizes patients’ medical records, ensuring files are complete, accessible, and properly stored. The role prepares, scans, verifies, and indexes records within 24 to 48 hours of discharge; reconciles discharged records and follows up on missing documents; and ensures physicians have properly signed, dated, and completed records before they are signed off as complete. The position also arranges and stores records from the skilled nursing facility, conducts periodic chart audits and reviews, handles scanning, photocopying, and faxing, and answers telephone questions about patients’ medical records. This is a full-time position, Monday through Friday, 8 a.m. to 4:30 p.m., at NHC Johnson City in Johnson City, Tennessee.

Requirements

Certification as a CCA, CCS, CPC, RHIA, or RHIT is required.

Nice To Haves

A minimum of one year of experience in medical records is preferred, preferably in a long-term care setting.

Responsibilities

Maintain patients’ files and ensure they can be retrieved upon request or for scheduled appointments.
Perform clerical tasks required to maintain patients’ medical records.
Reconcile discharged records and follow up on missing documents.
Prepare, scan, verify, and index patients’ records within 24 to 48 hours of discharge.
Ensure physicians properly sign, date, and complete patients’ records before signing them off as duly completed.
Arrange records from the skilled nursing facility in proper order, set them up for storage, log them into the system, and place them in storage containers.
Scan and photocopy records.
Receive and send information by fax.
Conduct periodic medical record chart audits and reviews.
Answer telephone calls and questions regarding patients’ medical records.
