The Certified Home Health Aide (CHHA) will be responsible for providing or assisting with ADL activities, including personal hygiene, grooming, mouth, skin, and nail care. They will assist with toileting, positioning, and transfer activities, including frequent turning and positioning of bed-bound residents/patients. The CHHA will also assist with prescribed exercises and Range of Motion (ROM) activities as taught by the Hospice Nurse, and perform light housekeeping of the resident's personal unit or room, including changing bed linens and patient laundry. Feeding and assisting with meals, obtaining vital signs (temperature, pulse, respirations) under the supervision of the Hospice Nurse, and reporting any changes in the resident's condition to the Hospice Nurse (and facility nurse if applicable) are also key responsibilities. Documentation of these reports is required. Additionally, the CHHA will assist or remind patients to take medication in compliance with regulatory guidelines.
