About The Position

Provides personal care and rehabilitative services and assists in activities
of daily living as directed by the Director of Clinical Services or the
supervising Registered Nurse.
Participates in the development and implementation of a realistic
goal-oriented plan of care.
Provides nutritional support by preparing grocery lists and
meals, assisting with feeding and fluid intake, keeping record of nutritional
intake.
Assists clients in administering medication.
Assists with personal care functions including grooming, bathing, toileting,
dressing wounds, taking simple vital sign measurements and recording.
Helping clients exercise, move around the house and transfer them
when required.
Accompanies clients to medical appointments.
Assists in socialization and recreation activities.
Helps with personal housekeeping duties

Requirements

• NYS Home Health Aide Certificate
