About The Position

Home & Health Care Management is the oldest home health care agency in Northern California, specializing in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients. This role is for a Certified Home Health Aide in California interested in working with an interdisciplinary team of health professionals helping individuals who have transitioned from a hospital to home care setting and assisting them to remain in the community. The ideal candidate enjoys helping clients achieve their personal goals of remaining in the community and wants to work with a team that inspires professional growth and shares the priority of providing quality case management and home health care.

Requirements

Certified Home Health Aide in California
Completed an approved Home Health Aide training course and received Certification as Home Health Aide from the State of California
Valid California Driver’s License, and eligible to be insured under our liability policy with a clean DMV report
Your own vehicle and proof of current auto insurance
Current CPR certification
Patient-interaction skills
Experience with ADLs
Experience with Hoyer Lifts
Ability to multi-task and perform duties in a timely and orderly manner

Responsibilities

Assists patient with personal care activities including bathing, oral hygiene, preparation and feeding of meals, dressing and undressing, ambulation, and other activities as needed
Performs duties as assigned in an accurate and timely manner
Notifies the office of any abnormal findings or conditions
