The role involves providing personal care, meal preparation, maintaining a safe home environment, documenting care, and reporting patient updates to RN Case Managers.
Requirements
- Active NJ CHHA certification in good standing
- Strong communication skills in English (written and verbal)
- Ability to lift, push, or pull up to 50 lbs
- Willingness to travel throughout Monmouth County
- Reliable transportation
Responsibilities
- Provide compassionate personal care: bathing, grooming, dressing, and mobility assistance
- Prepare simple, nutritious meals based on patients' dietary needs
- Maintain a clean and safe home environment
- Document all care using Celltrak
- Report patient condition updates to RN Case Managers
