Requirements
- Valid NJ CHHA license in good standing
- Clear communication skills in English (reading, writing, and speaking)
- Ability to lift, push, or pull up to 50 lbs
- Dependable transportation and willingness to travel within Bergen & Hudson Counties
Responsibilities
- Provide direct care including personal care (bathing, grooming, dressing, skincare, and mobility).
- Prepare simple, nutritious meals according to the patient's care plan.
- Ensure a safe, clean, and comfortable home environment.
- Record care and activities using CellTrak.
- Report condition changes to RN Case Managers for prompt action.
