Requirements
- Valid NJ CHHA license in good standing
- Clear communication skills in English (reading, writing, and speaking)
- Ability to lift, push, or pull up to 50 lbs
- Dependable transportation and willingness to travel within Essex County
Responsibilities
- Assistance with bathing, grooming, dressing, skincare, and mobility
- Creating simple, nutritious meals according to the patient's care plan
- Ensuring a safe, clean, and comfortable home environment
- Recording care and activities using CellTrak
- Reporting condition changes to RN Case Managers for prompt action
