About The Position

Bristol Hospice is seeking compassionate Certified Home Health Aides (CHHA) with a heart for patients and families. As a CHHA, you will play a critical role on the Interdisciplinary Group Team, working under the supervision of a Registered Nurse to provide personal care and promote patient comfort. Bristol Hospice is a nationwide leader dedicated to providing family-centered hospice services with the highest level of compassion, respect, and dignity.

Requirements

  • Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California.
  • Must have completed a minimum of 75 hours of classroom and supervised practical training.
  • Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order.
  • Willingness to submit to a criminal background check prior to hire and annually thereafter.
  • Current CPR certification.

Nice To Haves

  • One (1) year experience in community health/hospice or medical/surgical environments.
  • Ability to read and follow written instructions.
  • An understanding of hospice philosophy and comfort providing specialized care to the terminally ill.
  • Ability to work autonomously with little direct supervision.
  • Demonstrated tact, patience, and good personal hygiene.
  • Effective communication skills.
  • Strong organizational skills.
  • Understanding of plan of care documentation.
  • Flexibility in work hours to ensure patient care as per the plan of care.
  • Ability to empathize with the needs of the ill, injured, frail, and impaired.
  • Comfortable addressing issues of death/dying.
  • Calm demeanor when entering the patient's home.

Responsibilities

  • Perform personal care needs as assigned and respond promptly to patient requests, reporting any inability to perform tasks to the Case Manager.
  • Utilize positive communication techniques, maintaining confidentiality and ethical care standards.
  • Ensure patient safety by using equipment properly and safely.
  • Provide personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linens.
  • Assist patients with dressing and undressing.
  • Prepare nutritious meals, including shopping, as assigned.
  • Assist with feeding patients.
  • Record vital signs (temperature, pulse, respiration, blood pressure) as ordered and with demonstrated competency.
  • Provide skin care to prevent tissue breakdown.
  • Assess and report patient condition changes to the Case Manager, noting interpersonal dynamics within the household.
  • Assist with ambulation and exercises as instructed by the hospice nurse or therapist.
  • Perform range of motion and positioning, as directed.
  • Assist with self-administered medications by providing reminders, as permitted by state regulations.
  • Offer and assist with bedpan and urinal use.
  • Aid with light laundry needs if necessary.
  • Perform range of motion and other simple procedures as ordered and with demonstrated competency.
  • Provide respite for patient's family/caregiver when on-site.
  • Maintain patient's living area in a clean and orderly condition.
  • Adhere to organizational documentation, care procedures, and professional conduct standards.
  • Participate in quality assessment and performance improvement activities.
  • Respect patient and family/caregiver environments and personal needs.
  • Perform other duties as assigned.
