About The Position

Home & Health Care Management is the oldest home health care agency in Northern California, specializing in in-home care and case management for older adults, children, and individuals with disabilities. We utilize Medi-Cal, Medicare, and targeted health programs to find solutions for our clients. This role involves working with an interdisciplinary team to assist individuals transitioning from hospital to home care, helping them remain in the community. We are seeking Certified Home Health Aides who are passionate about helping clients achieve their goals and desire professional growth within a supportive team environment focused on quality case management and home health care.

Requirements

Certified Home Health Aide in California
Completed an approved Home Health Aide training course and received Certification as Home Health Aide from the State of California
Valid California Driver’s License
Eligible to be insured under our liability policy with a clean DMV report
Reliable transportation
Proof of current auto insurance
Negative TB skin test or chest X-ray
Ability to pass a pre-employment criminal background check
Current CPR certification
Patient-interaction skills
Experience with ADLs
Experience with Hoyer Lifts
Ability to multi-task and perform duties in a timely and orderly manner

Responsibilities

Assists patient with personal care activities including bathing, oral hygiene, preparation and feeding of meals, dressing and undressing, ambulation, and other activities as needed
Performs duties as assigned in an accurate and timely manner
Notifies the office of any abnormal findings or conditions
