About The Position

The Certified Home Health Aide (Hospice) at PARENTIS Hospice and Health is responsible for providing compassionate and personalized care to patients in their homes. The role involves supporting patients in managing their daily activities and ensuring their comfort and dignity during the end-of-life phase. The aide will work closely with a multidisciplinary team to deliver high-quality hospice care tailored to the needs of each patient.

Requirements

  • High school diploma or equivalent.
  • Completion of a state-approved Certified Home Health Aide training program.
  • Minimum of 1 year of experience working as a Certified Home Health Aide, preferably in a hospice setting.
  • Experience in providing compassionate care to patients with terminal illnesses.
  • Strong understanding of hospice care principles and practices.
  • Ability to follow a care plan and provide personalized patient care.
  • Excellent communication skills, both verbal and written, to interact effectively with patients, families, and healthcare team members.
  • Proficiency in basic patient care techniques, including bathing, dressing, and mobility assistance.
  • Ability to work independently and manage time effectively in a home setting.
  • Empathy, patience, and a genuine desire to help improve the quality of life for patients.
  • Basic computer skills for documentation and communication purposes.

Responsibilities

  • Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility.
  • Monitor and report changes in the patient's condition to the hospice team promptly.
  • Provide emotional support and companionship to patients and their families.
  • Administer prescribed medications under the supervision of a registered nurse.
  • Maintain accurate and timely documentation of patient care activities.
  • Ensure a safe and clean environment for patients by adhering to infection control protocols.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Educate patients and families on hospice philosophy and care practices.
  • Respect patient confidentiality and uphold ethical standards in all interactions.
