Certified Home Health Aide (Caregiver), ElderOne (Part-Time, Days)
About The Position
Compassionate, patient focused and dependable professional Critical thinking skills, decisive judgment and the ability to work with minimal supervision Exceptional documentation and communication skills Ability to read, write and speak English, ability to follow verbal and written instructions This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.
Requirements
- Candidates must meet one of the following:
- Active Home Health Aide (HHA) certification
- HHA with a lapse of more than two years of employment (must complete written exam and 1-day HHA Skills Competency Class within 30 days of hire)
- Certified Nursing Assistant (CNA) willing to complete a written exam and 1-day HHA Skills Competency Class within 30 days of hire
- Out-of-state HHA or Nurse Aide training with documented competency evaluation
- Nursing student (Fundamentals of Nursing completed within the past 24 months)
- U.S. Military-trained medic or medical technician
Nice To Haves
- High school diploma or equivalent
- 1 year experience working with the frail elderly population

Responsibilities
- Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands
- Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly
- Documentation. Accurately and completely document client care on activity sheet
- Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements
