Certified Diabetes Educator
About The Position

This role educates patients on self-care involved in the diagnosis of diabetes and collects patient data to monitor health status and proactively manage the patient's condition. The Certified Diabetes Educator (CDCES) provides initial and ongoing assessment, education, goal setting, and behavioral interventions to promote diabetes self-management skills and patient empowerment. They ensure patients and families understand self-care skills including healthy eating, medication management, physical activity, glucose monitoring/pattern management, problem-solving, and risk reduction. The CDCES demonstrates knowledge of pathophysiology, social determinants of health, and evidence-based standards. They support quality improvement to effectively maintain program recognition for an effective Diabetes Self Management Education program and collaborate with the multidisciplinary team, referring provider, and other healthcare providers related to diabetes care. The role also involves providing nutrition therapy and monitoring the nutritional care of clients/patients of all ages, recognizing, evaluating, and providing for nutritional needs based on cultural, ethnic, and religious beliefs. An individualized education and care plan is developed based on assessment, and patient needs are evaluated with appropriate management strategies suggested, including insulin management and diabetes technologies. Training and support are provided to patients and families related to diabetes management technologies (continuous glucose monitors and insulin pumps). The CDCES will maintain certification on insulin pumps based upon location requirements and stay abreast of current evidence-based practice and facility protocols related to diabetes care management. The role may also provide education or community support related to diabetes management or prevention and is responsible for working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.
Requirements
- Minimum of a bachelor’s degree in dietetics or food and nutrition through an Accreditation Council for Education in Nutrition and Dietetics (ACEND) accredited program. For graduates after 2024, a Masters degree is required per ACEND.
- Clinical nutrition experience through an approved Coordinated Graduate Program or post graduate internship is required.
- Three years of experience as a Registered Clinical Dietitian.
- Must have a thorough knowledge of normal nutrition, diabetes self-management skills and medical nutrition therapy.
- Must be registered by the Commission of Dietetic Registration (CDR) and licensed as a Dietitian or Nutritionist in the state(s) of practice.
- Must hold and maintain certification as a Certified Diabetes Care and Education Specialist from the National Certification Board of Diabetes Care and Education Specialist or obtain that certification within three years when eligible.

- Ability to recognize, evaluate, and provide for nutritional needs based on cultural, ethnic, and religious beliefs.
- Develops an appropriate age, individualized education and care plan based on assessment.
- Evaluates patient needs and suggests appropriate management strategies, including insulin management and diabetes technologies.
- Provides training and support to patients and families related to diabetes management technologies (continuous glucose monitors and insulin pumps) as appropriate.
- CDCES will maintain certification on insulin pumps based upon location requirements.
- Maintains practice by staying abreast of current evidence-based practice.
- Follows facility protocols related to diabetes care management.
- May provide education or community support related to diabetes management or diabetes prevention.
- Responsible for working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.
