About The Position
The Diabetes Care and Education Specialist - RDN serves in an advanced role providing in-depth analysis of the patient with diabetes and providing specialized care and in-depth education for persons with diabetes and their families. This role involves serving as a team member in assessing needs, planning and providing education and care, evaluating outcomes, and coordinating follow-up care for persons with diabetes. The position is an outpatient diabetes educator role in a clinic setting, working under ADA accreditation, and covering all areas of Diabetes Self-Management Training (DSMT), along with pump and Continuous Glucose Monitoring (CGM).

Responsibilities
- Assesses patient's appropriateness for intensive insulin therapy.
- Provides diabetes self-management education and training, including evaluation of individual skills, support, and barriers to care.
- Assesses general diabetes knowledge, evaluates physical findings and anthropometric measurements.
- Teaches appropriate essential self-care behaviors to improve effective self-management of their disease, including healthy eating, physical activity, blood sugar monitoring, medication administration and adherence, problem-solving, healthy coping skills, and risk reduction behaviors.
- Develops diabetes management plans appropriate to the patient's immediate diabetes needs.
- Reviews and adapts clinical recommendations and care plans based on lab results.
- Serves as a resource for patients and families.
- Documents in accordance with established standards.
- Investigates and problem-solves patient care issues with the care team and facilitates change to promote quality outcomes.
- Enhances the quality of diabetes care through collaboration with other services and initiates referrals to appropriate support services.
- Works within the multidisciplinary team for diabetes education and management and collaborates with other disciplines to identify and resolve patient care issues.
- Conducts diabetes education through group classes as indicated.
- Participates in departmental initiatives to meet strategic goals.
- Precepts students as indicated.
- Mentors and trains new staff members, colleagues, and other members of the care team as applicable.
- Practices with a high degree of autonomy.
- Evaluates evidence-based best practice and shares findings with the team, including proposing adjustments to supporting reference materials and policies.
- Provides guidance to providers and other healthcare professionals.
- Participates in multi-disciplinary care teams and functions as the diabetes expert.
- Collaborates with inter/intra professional team members to achieve department goals and optimize service.
- Serves as Designated Point Person for new hires as indicated.
- May include specialty program management and program evaluation outcomes.
- Accountable for satisfying all job-specific obligations and complying with all organization policies and procedures.
