About The Position

We are seeking a dedicated and empathetic Certified Diabetes Educator to join our healthcare team. In this role, you will empower patients diagnosed with pre-diabetes, Type 1, and Type 2 diabetes to take control of their health through comprehensive self-management education, lifestyle modification, and clinical guidance. This position does require CDCES to travel to various clinics throughout the APHS System. Reliable transportation is required.

Requirements

• Current, active license as a Registered Nurse (RN), Registered Dietitian Nutritionist (RDN), Pharmacist, Exercise Physiologist or other allied health professional.
• Certified Diabetes Care and Education Specialist (CDCES) credential preferred (or eligibility to sit for the exam within a specified timeframe).
• Preferred minimum of 1–2 years of clinical experience in diabetes education, endocrinology, or outpatient care.
• Strong proficiency with modern diabetes technology (CGMs, insulin pumps, smart pens).
• Excellent interpersonal, verbal, and written communication skills with a compassionate approach to patient care.
• Ability to translate complex medical terminology into clear, actionable advice for patients of diverse backgrounds.
• Reliable transportation is required.

Responsibilities

• Maintain accurate documentation in the patient’s EHR based on established documentation procedures.
• Fulfill clerical responsibilities as assigned including but not limited to the following: obtaining lab/X-ray reports, hospital notes, and referral information. Complete forms/requisitions as needed; verify patient demographics.
• Maintain patient confidentiality and protect operations by keeping protected health information confidential and secure.
• Assist Medical Providers with communicating with patients regarding diabetic medications, diabetic symptoms, referrals, appointments, and paperwork.
• Ensure culturally and linguistically appropriate services.
• Represent the company professionally and positively to enhance and promote the core values and mission of the organization.
• Participate in mandatory in-services and drills, attend staff meetings and other trainings, including appropriate age specific educational trainings offered on regular basis for professional development.
• Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services. Maintain safe, secure, and healthy work environment by following and enforcing established policies and procedures and the ability to professionally encourage others to do the same to support a healthy team atmosphere. Maintain strict adherence to infection control guidelines established by the organization. Critical thinking.
