The Centralized Referral Coordinator performs tasks pertinent to the age of the patient (infancy to geriatric). At the written order from a CVHS Provider, they make outside referral appointments and serve as a liaison between providers and patients for outside referrals. This role involves maintaining a Referral Data Base, printing daily referral lists, and following a tracking system by following up on all referrals. The coordinator provides written documentation of appointments kept or not kept, and receives written or verbal documentation from providers for missed appointments. They also provide assistance with insurance forms as needed prior to making appointments, fax completed referral forms to the appropriate facility, and coordinate with the provider and pharmacy for appointments requiring special preparation. Additionally, they provide special instruction sheets as needed, support a team-based approach to patient-centered medical care, attend work as scheduled, and perform other duties as assigned.
