About The Position

The Centralized Referral Coordinator performs tasks pertinent to the age of patients from infancy to geriatric. At the written order from a CVHS Provider, this role makes outside referral appointments and serves as a liaison between providers and patients for outside referrals. The position involves maintaining a Referral Data Base, printing daily referral lists, and following a tracking system by following up on all referrals. It also requires providing written documentation of appointments kept or not kept, and receiving documentation from providers for missed appointments. Additionally, the role assists with insurance forms, faxes completed referral forms, coordinates appointments requiring special preparation with providers and pharmacies, and provides special instruction sheets as needed. The position supports a team-based approach to patient-centered medical care, requires regular attendance, and may involve other assigned duties.

  • Medical terminology a plus.
  • Prior experience in the medical profession.

Responsibilities

  • Provides assistance with Insurance forms as needed prior to making appointments.
  • Faxes completed referral forms to appropriate facility.
  • Coordinates with the provider and pharmacy, appointments requiring a special preparation.
  • Provides special instruction sheets as needed.
  • Supports a team based approach to patient-centered medical care.
  • Attends work as scheduled.
  • Performs other duties as assigned.
