Centralized Bereavement Coordinator Manager

About The Position

Oversee the day-to-day operations of the Centralized Bereavement Team, ensuring exceptional support for patients and families across multiple branches. Provide leadership by managing team performance, training staff, approving time and PTO requests, and assisting with overflow calls. Carry a caseload when needed, coordinate bereavement services, and maintain compliance with organizational policies and the Conditions of Participation (CoPs).

Requirements

  • High school diploma or equivalent required.
  • Formal training in life-threatening and chronic illness, grief, and loss.
  • Minimum 1-year clinical experience in a multidisciplinary setting
  • At least 1 year in a healthcare setting
  • Three or more years of team leadership experience.
  • Clinical experience in life-threatening and chronic illness, grief, and loss.
  • Counseling experience
  • Strong knowledge of terminal illness, hospice care, and psychosocial dynamics related to illness and death.
  • CPR certification.
  • Valid driver’s license and current automobile insurance.

Nice To Haves

  • Graduate degree preferred if providing counseling.
  • Undergraduate degree in a human services field
  • At least 1 year experience in hospice, home health, community care, or hospital.
  • 3 years experience in multidisciplinary setting.
  • Individual, family, and group therapy counseling experience.
  • Advanced counseling certifications or licensure.
  • Previous experience leading a centralized or multi-location support team.

Responsibilities

  • Direct daily workflow, including taking overflow calls, training team members, approving time-off requests, and dispatching coordinators for visits.
  • Participate in providing bereavement support through phone contact, correspondence, and other personalized services.
  • Complete bereavement assessments in alignment with CoPs and organizational policies.
  • Maximize access to resources such as volunteers, support groups, and community referrals.
  • Maintain accurate and timely bereavement charting and ensure team compliance with documentation standards.
  • Prepare monthly summary reports of activities, projects, and case closures.
  • Contribute to the hospice’s quality assessment and performance improvement initiatives.
  • Represent the organization in marketing bereavement services to the community and referral sources.
  • Provide consultation and training to bereavement staff in other offices.
  • Support the bereavement needs of staff and volunteers.
  • Collaborate with interdisciplinary team members to develop and coordinate family bereavement care plans.
  • Perform additional duties as assigned.
