About The Position

At Avalon Health Care Management, Inc., you'll be part of a corporate clinical team that supports skilled nursing facilities across the organization. This is an opportunity to use your MDS expertise beyond a single facility while helping drive quality outcomes, compliance, and reimbursement accuracy throughout the company. As a member of the Central MDS Team, you'll work alongside experienced clinical leaders and reimbursement specialists while supporting facilities across multiple markets.

Requirements

• Current RN license in good standing
• Previous MDS and skilled nursing facility experience required
• Strong knowledge of the RAI process and case management
• Ability to work independently and collaboratively with clinical teams
• Ability to pass required employment screenings

Responsibilities

• Lead the MDS and resident assessment process
• Ensure assessments and documentation are completed accurately and on time
• Coordinate care planning with the interdisciplinary team
• Support Medicare and Managed Care requirements
• Help drive quality outcomes and reimbursement accuracy
• Serve as a key resource for MDS, care planning, and compliance
