Jewish Family Services, Inc. is a nonprofit organization that provides supportive services focused on strengthening families, children, and individuals throughout the life cycle within the context of their unique needs. They offer a wide range of mental health and counseling services, social services, case management programs, and safe and affordable housing to nearly 2,500 individuals annually. Jewish Family Services serves adults, children, seniors, individuals with disabilities, chronic mental health problems, substance use disorders, and families with low household incomes. The CCS Mental Health Professional is responsible for providing day-to-day oversight and consultation to Care Coordination staff, conducting intakes, completing assessments, participating in and/or developing recovery and discharge plans, and evaluating service plans according to the requirements of the Comprehensive Community Services program. Additionally, the CCS Mental Health Professional is responsible for leading the Care Coordination team in developing treatment plans that meet CCS consumers’ unique needs and recovery goals, enabling them to achieve their individual highest level of independent functioning.
