About The Position

Apalachee Center, Inc. is best known for helping individuals and families of North Florida succeed in recovering from emotional, psychiatric, and substance abuse crises. Apalachee Center, Inc. provides comprehensive behavioral health services across 8 counties (Franklin, Gadsden, Jefferson, Leon, Liberty, Madison, Taylor, and Wakulla Counties). The purpose of CAT case management services is to assist youth and their families in gaining access to needed medical, social, educational, and other services. The primary goal of case management is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. Services and service frequency should accurately reflect the individual needs, goals, and abilities of each recipient.

Requirements

  • Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, health science, or a related human services field.
  • One-year full time or equivalent experience working with children experiencing serious mental illness preferred.
  • Current valid driver's license.
  • Maintain Apalachee Designated Auto Driver status.
  • Maintain current ADM Targeted Case Manager Certification.

Responsibilities

  • Complete comprehensive assessment of individual youth strengths and needs, including review of family factors important to successful youth functioning.
  • Conduct home visits prior to completion of the intake assessment process to evaluate the safety and well-being of the client in their residence, unless otherwise staffed with a supervisor and documented.
  • Develop individualized service plans with measurable goals and objectives in partnership with the youth and their families, addressing both youth and family factors.
  • Assist with the implementation of the service plan, emphasizing helping family members achieve autonomy in meeting youth and family needs.
  • Assist families with establishing a natural support system that provides ongoing support during and post-program participation.
  • Collaborate across providers and natural support systems, and advocate for and assist the youth and their family to build meaningful partnerships and address their needs.
  • Assist clients in gaining access to needed financial, transportation, employment, medical, social, education, and other services.
  • Follow up on the client’s services to determine the status and effectiveness of the service plan towards enhancing the client’s inclusion in the community.
  • Document all services provided in the client record in accordance with Client Record Handbook Guidelines, finalizing documentation in Avatar within 48 hours of service delivery.
  • Ensure progress notes are detailed and clearly reflect how the case manager’s efforts are linked to the services and goals in the client’s service plan and the client’s progress or lack of progress.
