About The Position

The professional registered nurse has the authority, responsibility, and shift accountability for the delivery of nursing care through the use of the nursing process. The Registered Nurse accepts accountability, applies competent clinical knowledge skills, and uses independent nursing judgment for care provided by self and by health care providers to whom care is delegated to achieve quality patient care outcomes. The Registered Nurse must demonstrate the knowledge and skills at the advanced beginner level necessary to provide care that is relevant to the patient(s) under their care, as well as apply principles of growth and development over the life span. The Registered Nurse also must possess the ability to assess and interpret patient data needed to identify each patient's requirements relative to his or her age-specific needs. This role provides professional nursing care for patients in an inpatient hospital setting.

Requirements

  • Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Obtain certification in Basic Life Support within 30 days of hire date.
  • Three (3) years of nursing experience
  • Knowledge of and appropriate application of the nursing process.
  • Knowledge of professional theory, practice and procedure.
  • Ability to communicate clearly with patients, families, physicians, and other employees.

Nice To Haves

  • Bachelor’s degree in Nursing (BSN) within 3 years of hire date.
  • Five (5) years of experience in palliative care.

Responsibilities

  • Promotes the continuum of care in inpatient and outpatient infrastructure.
  • Prepares and performs annual review of the program.
  • Coordinates the development and implementation of policies and procedures.
  • Directs and oversees Performance Improvement initiatives related to Palliative Care and identifies trends.
  • Coordinates data collection. Performs analysis of data and identifies trends.
  • Identifies opportunities for improvement in patient outcomes and participates in plans to enhance clinical outcomes and patient satisfaction.
  • Acts as a liaison for community support for Palliative Care, establishes and maintains relationships with community agencies, hospitals, sponsors, and support group.
  • Assists in the provision of palliative care and the performance of palliative care consultations
  • Evaluates and assesses patient medical condition through the use of histories, physical examinations, and review of previous medical records.
  • Works collaboratively with care management, chaplaincy, rehabilitative services, pharmacy, dieticians and house staff to promote comprehensive, coordinated palliative care for patients.
  • Works as a member of the multidisciplinary team providing specialist evidence based nursing care to patients and to support families and care givers.
