About The Position

This role involves working with clients to assess their needs, develop program plans, and support their successful reintegration into the community. The Caseworker will administer assessments, identify barriers, create individual program plans, make referrals, and assist clients with housing and release preparations. The position requires accurate data entry and adherence to program rules and standards.

Responsibilities

Administer or utilize current quantitative assessments of clients’ risk and criminogenic needs, including criminal history, education/employment/financial, family/social support, neighborhood problems, substance use, peer associations, and criminal attitude/behavior patterns.
Enter ORAS results into Gateway and ORION accurately and timely.
Identify clients’ barriers to successful program completion and respond to barriers such as age, education level, medical, mental health, transportation, housing, etc.
Complete and/or review intake/release packets/PREA screening results.
Assist clients with becoming more positive, productive, and prosocial members in the community.
Act as an advocate for clients and hold clients accountable to program rules.
Administer assessment tools properly and ensure updates are completed as needed.
Complete individual program plan (IPP) using specified criteria/standards.
Make referrals based on client’s ongoing needs and recommended targeted dosage hours as reflected in the Community Plan.
Ensure post-tests and/or exit evaluations are completed prior to client’s release.
Assist clients with making suitable housing arrangements.
Prepare clients for release from the program with an approved and appropriate Community Plan.
