About The Position

Provide direct services to consumers to assist in the identification and resolution of areas of need. Facilitate the use of community resources. Establish and periodically reassess care plans making modifications as needed. Monitor an array of community services available to consumers.

Requirements

  • Bachelor's degree in sociology, social work/social welfare, psychology, gerontology or other of the behavioral sciences
  • One year of experience preferred.
  • Good communication skills both verbal and written.
  • Good outreach skills and knowledge of community resources and funding source regulations needed.
  • Must possess knowledge of Microsoft Office (Outlook, Word, Excel, and Power Point) and Microsoft Teams.
  • The ability to operate a computer in a Windows environment is essential.
  • Must have enough knowledge to successfully complete the technical skills of the position.
  • Must be able to travel.
  • Must have a valid PA driver’s license, full-time access to private automobile and auto insurance as required by Agency.
  • Must meet Immigration Law requirements.
  • Act 33, 34 & FBI clearances required prior to hire.
  • Updated clearances must be submitted every 60 months.

Nice To Haves

  • Familiarity with Zoom, PDF editing software, as well as experience effectively working remotely are desired.

Responsibilities

  • Provides casework services to clients as assigned and completes initial and re-evaluating NAT.
  • Develops care plans and goals to meet needs of clients within program limits.
  • Determines and monitors cost-sharing requirements for consumers.
  • Monitors each client through face-to-face contact to assess care progress, monitors the adequacy of service delivery and coordinates services according to an appropriate care plan.
  • Records contact information in case files in line with established procedures.
  • Informs clients of available services and develops with clients care plans designed to meet their needs, including the provision of informed recommendations to clients regarding service objectives.
  • Utilizes and coordinates community resources to assist clients in activities of daily living.
  • Sends necessary information to service providers to ensure prompt possible service delivery.
  • Documents and maintains client records as required in line with established procedures and completes reports monthly.
  • Performs data entry and assessment activities on Wellsky Aging & Disability database.
  • Participates in staff meetings, in-service trainings, and individual consultations when scheduled.
  • Advocates for the elderly at every opportunity.
  • Performs other duties as properly assigned by Supervisor and others in appropriate line of authority.
  • Must be able to transport, operate, and protect computer equipment in the field. (i.e., laptop computer)
  • Safeguards, secures and otherwise protects GECAC property, equipment and supplies.
  • Seeks out isolated persons who may have unmet needs and are unfamiliar with the AAA.
  • Determines and monitors eligibility requirements for the Caregiver Support Program.
  • Provides casework services to clients as assigned and completes initial and re-evaluating CAT (for CSP).
  • As needed and requested, may attend OAPS training to provide backup for Older Adult Protective Services, if the need arises.
  • Maintains HIPAA regulations and Agency confidentiality standards.
