About The Position

Under the general direction of the IDD Authority Director, the Service Coordinator Mentor is responsible for mentoring service coordinators with performance of service coordination functions and activities. Functions and activities include but are not limited to; planning, training, providing consultation and problem solving, demonstrating, collaborative support, positive and non-evaluative feedback, and emotional support. Position requires moderate to extensive travel to consumer homes and flexible scheduling. A cellular phone is required. The Service Coordinator Mentor provides Service Coordination functions as outlined by Texas Administrative Code, 40 TAC, Chapter 2, Subchapter L: Service Coordination for individuals with intellectual and developmental disabilities. Duties outlined under this code include: coordinating the development of each assigned consumer’s Person Directed Plan, coordinating and monitoring services, monthly contact with each assigned consumer, and working closely with private and public providers of Home & Community-Based Services, Texas Home Living Services, and General Revenue Services. Additional duties include building relationships and partnerships with consumers, and family members; assisting consumers in determining outcomes; incorporating the values of Person Directed Planning; referring to community resources; maintaining Medicaid eligibility; monitoring services and supports; resolving problems or concerns; and working within timeframes.

Requirements

  • Bachelor or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
  • Valid Texas Driver’s License required.
  • Must not have more than 3 moving violations in the last 3 years.
  • Must be insurable and remain insurable on Center’s insurance.
  • Must be able to pass a background check.
  • Must be able to complete all MHMR Concho Valley training.
  • A cellular phone is required.

Nice To Haves

  • Ability to recognize barriers and coordination issues between service systems, and to advocate effectively for individuals served.
  • Possess and maintain organizational and typing skills.
  • Ability to communicate both verbally and in writing.
  • Ability to work as a team member in a diverse environment interfacing with both public and private providers.
  • Effectively communicate and work with staff, consumers, family members, visitors and other fellow employees in a manner that demonstrates knowledge of cultural diversity.
