About The Position

This position is responsible for monitoring an assigned caseload of clients participating in a short-term life management program. The Case Worker will establish program plans and goals, evaluate client progress through routine meetings, and serve as an advocate for clients to acquire necessary services. Key responsibilities include interviewing applicants to determine eligibility, conducting needs assessments, referring clients to other agencies when appropriate, and preparing and maintaining accurate case records and logs. The role also involves entering client information into the Homeless Management Information System (HMIS) and mentoring clients by providing positive examples in areas such as hygiene, personal appearance, records retention, budget planning, and scheduling.

Requirements

  • Ability to meet attendance requirements.
  • Ability to read, write, and communicate the English language.
  • Ability to communicate clearly and effectively both orally and in writing.
  • Ability to think clearly and quickly in order to maintain control of client caseload.
  • Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized.
  • Two years of college coursework from an accredited college or university in Social Work, Behavioral Science or a related field AND Two years’ experience working in a social or public service environment with experience assisting the public OR Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
  • Requires a level 2 background screening.

Responsibilities

  • Monitor assigned caseload of clients participating in a short-term life management program.
  • Establish program plan/goals and evaluate client's progress by conducting routine meetings with client and/or staff.
  • Serve as an advocate for clients to acquire services that will enable them to functionally cope with their environment.
  • Interview applicants to determine eligibility for program enrollment based on established guidelines and requirements.
  • Conduct needs assessment, obtain pertinent information and establish program goals for eligible clients.
  • Refer applicants to other agencies if not appropriate for program.
  • Prepare and maintain case records and logs on all assigned clients.
  • Ensure the accuracy and completeness of case records and logs.
  • Enter pertinent information into the established Homeless Management Information System (HMIS).
  • Serve as a mentor and provide positive examples of proper hygiene, personal appearance, records retention, budget planning, scheduling, etc.
