About The Position

Under the supervision of the Shelter Director, the Program Case Worker will coordinate support services and collaborate with community partners to assist clients in the Short Term Post-Hospitalization and Housing Program obtain housing and other needed community services within 6 months.

Requirements

  • 21 years old (required)
  • High school diploma or equivalent (required)
  • CA driver’s license (required)
  • Have or be willing to obtain certification in First Aid and CPR
  • Experience in electronic records data input and management
  • Experience with disadvantaged populations, whether through work, volunteering or being a member of a disadvantaged population (required)
  • Experience in resource development for homeless populations
  • Possess maturity and confidentiality
  • Proficient in Microsoft Office Suite and Outlook
  • Team Player
  • Ability to stay organized and multi-task

Nice To Haves

  • Experience and/or training in HMIS is (preferred)
  • Education in social services/substance abuse (preferred)

Responsibilities

  • Assist the Housing specialist as needed to provide program recipients intensive housing assistance including all or part of a comprehensive assessment, case management, service authorization, information, and referral necessary for program recipients to develop and pursue an effective a mutually agreed upon housing plan to assist clients in breaking out of homelessness.
  • Provide expert case management for clients with varying degrees of vocational, social, cultural, and/or medical impairments hindering self-sufficiency. This includes interviewing clients upon enrollment in the program and developing an individualized case management plan with each client, meeting together regularly to monitor progress.
  • Maintain client case records, files, and required forms per program procedures and protocols.
  • Enter data into HMIS and WellSky.
  • Be a resource specialist with knowledge of resources and ability to refer guests to specific, appropriate services including but not limited to housing, employment, mental and physical health services, drug and alcohol rehabilitation and recovery, Social Security, Disability, and other support services.
  • Establish and maintain a working relationship with all referring agencies.
  • Monitor contractual requirements as related to the program.
  • Complete required statistics and reposts, and log pertinent information in a timely fashion.
  • Other related duties as required.
