About The Position

This position serves as a Complex Care Coordinator within the Department of Behavioral Health and Developmental Disabilities (BHDD), Office of Intellectual and Developmental Disabilities (OIDD). The role is dedicated to providing compassionate and accessible care to the community, assisting individuals and families in navigating challenges and achieving long-term wellness. BHDD-OIDD is part of the state's behavioral health and developmental disabilities agency, providing services for individuals with intellectual and developmental disabilities, mental health conditions, and substance use disorders through a holistic, streamlined, and coordinated approach. OIDD assists in the planning, developing, coordinating, and funding services for South Carolinians with severe, lifelong disabilities including intellectual disability and related disabilities, autism spectrum disorder, traumatic brain injury, spinal cord injury, and similar disabilities.

Requirements

  • A bachelor's degree or graduate degree from an accredited college or university.
  • At least one year of experience working with the target population.
  • Experience in the OIDD service delivery system.

Nice To Haves

  • Knowledge of interagency case management principles and practices.
  • Familiarity with the populations served by OIDD (individuals with intellectual disabilities, related disabilities, brain injury, spinal cord injury, and similar disabilities) as well as available services/supports.
  • Knowledge of Medicaid State Plan and OIDD operated Home and Community-Based (HCB) Waiver services.
  • Ability to interpret and apply policies and procedures.
  • Excellent verbal and written communication skills, including the ability to communicate effectively.
  • Computer literacy, including the ability to use standard office software and electronic documentation systems.
  • Ability to exercise judgment and discretion as well as establish and maintain effective working relationships.

Responsibilities

  • Provides comprehensive case management services for assigned cases.
  • Assists with implementing needed services and supports for stabilization.
  • Authorizes services as approved on the Support Plan.
  • Facilitates transfer to identified case management provider once the case has been stabilized.
  • Documents activities as required and provides requested updates to supervisor.
  • Other case management functions as assigned.
