Case Worker - Adult Outpatient

Through our dedicated, experienced and skilled clinicians, we provide patients and their families with the highest quality of personalized care. Programs are offered for children, adolescents and adults struggling with mental illness as well as offering services specializing in Anxiety, Eating Disorders, and Geriatric Services. Job Summary Provides routine social services as a member of a multi-disciplinary team, including obtaining social histories, providing counseling and guidance, referrals to agencies that respond to the psycho-social needs of patients/clients and families. As appropriate identifies and refers, or seeks supervision on complex cases. Reports to Director of department or program, or designee.
Requirements
- Bachelor’s Degree in a human services related field or Bachelor’s degree with human service work experience
- Experience working in related human service setting with similar age-group and/or substantive area
Responsibilities
- Provides routine social services as a member of a multi-disciplinary team
- Obtaining social histories
- Providing counseling and guidance
- Referrals to agencies that respond to the psycho-social needs of patients/clients and families
- Identifies and refers, or seeks supervision on complex cases
