About The Position

Responsible for overseeing all case work services. Ensures the completion of all required paperwork and full compliance with all City and State RHY regulations and standards.

Requirements

Bachelor degree in a related field is required
Ability to work efficiently and independently.

Responsibilities

Complete Initial Service Plan Review & Case Conferences for clients/families & facilitate additional family meetings and/or program visits.
Coordinate services identified in the Initial Service Plan & subsequent reviews to assist clients in meeting their short & long-term goals.
Complete home assessments for clients who will be utilizing weekend passes.
Report any suspected abuse or neglect to Justice Center/State Central Register.
Become DYCD Connect trained & proficient.
Oversee Counseling Specialists regarding their specific responsibilities supporting case management services.
Provide aftercare services to clients for 90-days post discharge.
Maintain case records & ensure necessary documents are present on all current residents in their 90-day aftercare period.
Facilitate weekly force field/team meeting.
Conducts initial intake interviews of prospective residents in the absence of Intake Coordinator.
Work collaboratively with Program Coordinator, Counseling Specialists and Intake Coordinator to ensure quality of service delivery.
Attend & Participate in QI/QA & program admin meetings
Participate in staff and/or team meetings.
Perform other duties as assigned.
Responsible to maintain a caseload.
