About The Position

Child and Family Services Preventive Services helps families navigate behavioral challenges and traumatic experiences through homebased intervention and support. Upon referral from Erie County, our team of Case Planners helps families develop the skills and connect with resources to help prevent out of home and foster care placements for their children. Case Planners work directly with family members using an evidence-based Motivational Interviewing Counseling Model to engage and build relationships and make linkages to other community-based services.

Requirements

  • A Bachelor’s Degree
  • Experience working with children and families
  • Scheduling flexibility in order to meet the needs of families that fall outside of the traditional 9am-5pm window
  • A Valid NYS Driver’s License, reliable vehicle and adequate insurance

Nice To Haves

  • Have experience providing case management services
  • Are fluent in two or more languages

Responsibilities

  • Traveling to provide casework and case management services to families in their homes to assess and monitor safety, imminent risk, and well-being of children
  • Conduct evidence-based assessments and assess family needs in order to establish stable functioning and a safe environment
  • Collaborate with parents to develop a plan to encourage and strengthen a meaningful relationship between the parents and children
  • Retain up-to-date case records and progress notes in electronic database in accordance with NYS regulations
  • Engage in regular contact with Erie County Case Workers and coordinate with other agencies and community service providers
  • Plan and arrange for case conferences and service plan reviews
  • Maintain casework contacts as mandated by county regulations, a minimum of two face-to-face contacts with family members especially all the children
  • Collaborate with other members of the Preventative Services team, attend weekly meetings, offer expertise and resources, and actively participate in case conferences
  • Model and practice sensitivity, fair treatment, and acceptance of diversity in all interpersonal interactions with both internal and external customers
  • Maintain positive working relationships with program staff and others within the agency
