The Case Manager will recruit and maintain a caseload of up to 50 participants, conducting monthly contact with each and serving 100 participants within a program year. This role involves establishing and maintaining cooperative, trusting relationships with individuals, families, and clinical/community-based organizations, using caring and non-judgmental language. The Case Manager will identify and share appropriate Healthy Start information, encourage referrals to and from clinics and community organizations, and utilize various outreach methods to engage diverse individuals and groups. Responsibilities include completing required assessments, assisting participants/families with goal creation for an Individual Family Service Plan (updated quarterly), and providing home-based services including health education, breastfeeding support, doula services, and other case management activities. The role also emphasizes promoting father/partner engagement, educating on well-child visits and immunizations, discussing parent-child interactions, screening for substance abuse and postpartum depression, assisting with Family Reproductive Plans, educating on child development and safety (including safe sleep), identifying children with special needs, and advocating for culturally and linguistically appropriate services. The Case Manager must understand and adhere to organizational policies, contribute to continuous quality improvement, maintain client records, compile and enter data into ETO, provide data for program reporting, inform the team of available resources, develop a consistent visiting schedule, and comply with HIPAA regulations. Additional duties may be assigned.
