About The Position

The Case Manager position requires a high level of motivation and genuine concern for families and the community. HOU Case Managers will follow program philosophies to guide the implementation of Individual Service Plans that assist families in achieving their personal and employment-related goals. Case Managers are responsible for linking work-eligible residents to employment and educational opportunities to enhance self-sufficiency. HOU's mission is to ensure that residents are treated fairly and experience minimum physical and emotional stress during the relocation process while ensuring that our clients' projects are completed in a timely and cost-effective manner.

Requirements

  • Must have BSW/MSW or 3 years of related academic and work experience
  • Experience working collaboratively with low-income families
  • Problem-solving and organizational skills
  • Proficient in Microsoft Office
  • Excellent written and verbal communication skills; ability to establish reports
  • Ability to motivate others towards achieving their goals
  • A strong sense of respect for confidentiality involving both clients and fellow employees
  • Must be able to pass employment criminal background screening

Responsibilities

  • Meet (through office and home visits) and establish working relationships with all heads of households on-site resident 18 years of age and older, to negotiate and develop Individualized Services Plans.
  • Provide/link residents to appropriate job readiness activities (skill/ability assessment, resume building, basic computer skills, employment search resources, interview skills, dress for success).
  • Refer residents to educational opportunities, including college, specific skills training, etc., by maintaining contact with ESOL and GED agencies, college and university representatives, technical training schools, etc.
  • Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/ success of referral.
  • Participate in weekly case management meetings and provide regular caseload progress reports to the Director of Resident Services and or other team members.
  • Attend case management meetings with agencies serving families on caseload, as necessary.
  • Participate in all required meetings and staff training opportunities
  • Input and maintain accurate and complete data for all residents in efforts and outcomes (ETO) case management software
  • Complete monthly reports no later than the 5th of each month
  • Conduct office/home visits with residents on caseload
  • Bilingual Spanish speaking a plus
  • Other duties as assigned
