The Shelter Case Manager provides onsite case management services, including creating individualized goals to assist clients in swiftly exiting shelter, documenting client progress in a database within 48 hours, identifying and addressing client barriers to housing and/or employment, and connecting clients to benefits, eligible housing resources, and community referrals. This role involves collaborating with the program team to increase positive shelter exits for guests.
