About The Position

A Step Ahead Partial Hospitalization Program- Bridgeton Monday-Friday Responsibilities: Assist families in completing behavior rating scales. Assists the clinician in the gathering of information from children, parents, day-care providers, school personnel, counselors, etc. at admission/Intake. Acts as a liaison between referral sources, the patient and family and the program. Follow-up with referral sources regarding intake cancellations/no shows and to give updates on waiting lists. Assist clinicians in case management duties such as scheduling patient appointments, requesting documentation, etc. Complete discharge follow-up phone calls. Participates in the development of the treatment plan where appropriate. Provide case management services including: education, coordination, referral administration, and monitoring. Facilitates and supports psycho-educational groups. Accurately records daily patient progress in the medical record as well as completing other required paperwork. Assist in helping staff with daily occurrences such as de-escalating crises, helping dispense supplies, resources, lunch, organizing information for resource room, etc. Assist in basic data gathering on performance improvement initiatives, quality assurance, and/or social problems. Provide training to community groups, agencies and other professionals about health care issues and community problems. Active participation in coordination of special program events. Maintain familiarity with professional and self-help systems in the community and will assist the client in using those services when necessary. Participate in weekly supervision. Rides along with the van driver to transport patients to and from program as needed. Complete certification in Handle with Care behavior management systems. Able to maintain strict confidentiality and professional/ethical standards required by state and federal statues and by those standards accepted for counseling and social work place.

Requirements

  • MUST have a bachelor's degree in human service-related field (master's preferred).
  • MUST have a year of case management experience with the youth in a behavioral health agency or institutional setting.

Nice To Haves

  • Masters in a human service-related field (social work, mental health counseling, human development, psychology, sociology, etc.) preferred
  • Certification in CPR and first aid within 90 days of hire.
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work with a diverse team.
