This full-time, hybrid Case Manager position requires travel to community-based meetings multiple times per week and remote work from a home office. In-person meetings are required, so applicants must reside within the State of Indiana or within 10 miles of the state line. Case managers will work with Individuals on two Medicaid Waivers: Family Supports Waiver (FSW) and Community Integrated Habilitation Waiver (CIH).
