About The Position

The Social Services Case Manager collaborates with physicians, nurses, and multidisciplinary team members to develop a plan of care for assigned patients from admission through discharge. This role ensures patient is progressing towards desired outcomes by monitoring patient care through assessments and evaluations.

Requirements

Masters: Social Work (Required)
BLS American Heart E-Card (BLS AHA ECARD) - American Heart Association (AMERICAN HEART)
BLS American Heart RQI E-Card (BLS AHA-RQI ECARD) - American Heart Association (AMERICAN HEART)
BLS Cert American Heart_non ecard (BLS AHA) - American Heart Association (AMERICAN HEART)
BLS Certification Grace (BLS GRACE) - Employee Grace Period for Essential Credential (GRACE)
BLS Cert Red Cross (BLS RC) - Red Cross (RED CROSS)
Masters degree in Psychology, Mental Health, Marriage and Family, and/or related social services field.
Licensed Clinical Social Worker license is required for the following areas: Dialysis, Transplant, Cystic Fibrosis and mental status exams in the Emergency Department.

Nice To Haves

Licensed Clinical Social Worker in Florida is preferred.

Responsibilities

Assesses and initiates crisis interventions and Baker Act if necessary.
Collaborates with the interdisciplinary team and consults with nursing staff to regularly evaluate the patient's current and ongoing needs.
Assesses and responds to patient/family needs by coordinating efforts of other team members.
Identifies and resolves barriers that hinder effective patient care.
Collaborates with governmental agencies (such as DCF, APS, CMS, and KePro) to ensure safe post-acute care.
Participates in discharge planning including coordinating patient transfers to other facilities and coordination of community.
Works with patients/families to navigate through the healthcare system (post-acute choices, insurance plans, etc.) to coordinate.
Organizes and facilities care conferences with patients, families, representatives, and other members of the care team.
