The Colorado Coalition for the Homeless is dedicated to preventing homelessness and creating lasting solutions for individuals, families, and children facing or at risk of homelessness across Colorado. As a Case Manager I, you will collaborate with case managers, supervisors, integrated health teams, and community partners to foster long-term solutions for individuals and families experiencing or who have previously experienced homelessness. Your responsibilities will include developing and implementing care plans, coordinating services and resources, monitoring client progress, providing advocacy and support, facilitating referrals, and maintaining case documentation and data collection. Services are delivered using a strengths-based, trauma-informed approach, incorporating principles of motivational interviewing, stages of change, housing first, harm reduction, and cultural responsiveness. This role emphasizes outreach, facilitating referrals to mainstream services and housing, and crisis intervention.
