About The Position

Rosecrance is seeking dedicated individuals to join their team as Case Managers for the Mental Health Pathways program. This role involves providing mental health rehabilitation services and supporting adults and families to reduce hospitalizations and crisis episodes, while increasing community functioning to achieve rehabilitative, resiliency, and recovery goals. The Case Manager will ensure continuity of program services ethically and legally, participating as a team member to integrate recovery principles into clients' daily lives.

Requirements

• Bachelor degree in Psychology, Counseling, Social Work, or other related field, or equivalent experience and training.
• Valid driver’s license and ability to meet Agency insurance requirements.
• Must have own transportation.
• Self-starter with ability to work independently and with a team.
• Computer proficiency required.
• Ability to navigate and document in electronic healthcare records required.
• Ability to accurately complete required documentation within the time frames.

• Professional Certification (e.g., CADC, CRSS, or CPS)

Responsibilities

• Provide mental health rehabilitation services and supports to adults and families.
• Decrease hospitalization and crisis episodes.
• Increase community functioning to achieve rehabilitative, resiliency, and recovery goals.
• Provide continuity of program services in an ethical, legal, and moral manner.
• Participate as a team member in the delivery of mental health services.
• Facilitate the integration of recovery principles into daily living situations.
• Motivate and problem-solve with staff regarding effective clinical practices.
• Be self-motivated, seek improved performance, and problem-solve while meeting complex client needs.
• Adapt verbal and written communication to the language and style of the client and their family.
• Complete all necessary delivery of service and documentation within required timeframes to maintain program fidelity.
• Undertake primary case management responsibilities for designated clients.
• Work with clients and guardians to develop a comprehensive multidisciplinary treatment team.
• Participate in crisis assessment process for designated clients.
• Ensure client service records are maintained consistent with regulations.
• Maintain communications with the multidisciplinary treatment team.
• Coordinate services from a variety of community resources.
• Utilize a variety of treatment concepts, evidence-based practices, and service modalities.
• Familiarize with court proceedings and be available to make court appearances on behalf of Rosecrance clients.
• Exhibit actions and make decisions that align with the Rosecrance Code of Conduct and within the framework of the law.
• Serve as a member of the team, participating in all team meetings, duties, and activities as assigned.
• Serve as a role model to others and demonstrate excellent company stewardship.
