About The Position

Terros Health is seeking a compassionate and professional Case Manager to join our team at the 27th Ave Health Center in Phoenix, AZ. This full-time position involves providing effective case management, clinical perspective, and service coordination for families in accordance with the Arizona Practice Model 12 Principles. The role supports individuals with serious mental illness/general mental health in improving their daily functioning, independence, and overall quality of life through active engagement and goal-directed services. Requirements

  • High School Diploma or GED and a minimum of 1.5 years of behavioral health experience, OR Associate's degree in a related field, OR Bachelor's degree in a non-behavioral health field with 1 year of behavioral health experience, OR Bachelor's degree in a behavioral health - related field
  • Ability to engage individuals using a non-judgmental, person-centered, and recovery-oriented approach
  • Strong communication and interpersonal skills, including active listening and clear documentation
  • Ability to provide active, treatment-focused services that support skill-building and improved functioning
  • Basic understanding of serious mental illness, co-occurring conditions, and community-based care
  • Ability to manage time, prioritize tasks, and maintain productivity expectations
  • Ability to respond to changing situations, including crisis or high-risk scenarios, with appropriate support
  • Basic computer skills and ability to utilize an electronic health record (EHR)
  • Valid Arizona Driver’s License and ability to meet organizational driving requirements
  • Must be at least 21 years of age
  • Valid Level 1 Fingerprint Clearance Card (or ability to obtain within required timeframe)
  • Successful completion of background check
  • Ability to work in both office and community-based settings

  • Experience working with diverse populations in behavioral health, including individuals with serious mental illness, co-occurring disorders, and varying levels of care needs.
  • Experience providing community-based or field-based services
  • Familiarity with Motivational Interviewing or other engagement techniques
  • Knowledge of local community resources and systems of care

Responsibilities

  • Provides direct, face-to-face services in the home, community, or clinic that support treatment goals.
  • Uses each interaction to teach, coach, or support skill-building related to daily living, health, and recovery.
  • Engages individuals using strength-based, trauma-informed, and recovery-oriented approaches.
  • Proactively re-engages individuals who are disengaged, difficult to reach, or at higher risk.
  • Engages regularly with individual to identify and implement goals and interventions.
  • Supports individuals in building skills related to independent living, housing stability, employment, relationships, and self-care.
  • Helps individuals navigate real-life challenges and practice skills in real-world settings.
  • Focuses on progress over time, not just service completion.
  • Encourages increasing independence while avoiding long-term dependence on services.
  • Develops and implements individualized service plans that are clear, realistic, and focused on functional outcomes.
