About The Position

Established in 1926, Family Service League is a social services agency who provides comprehensive services to individuals, children, and families to improve the quality of their lives at home, in the workplace, and in the community. With more than 60 social service and mental health programs in over 20 locations, Family Service League is committed to serving Long Island’s most vulnerable citizens. Family Service League is seeking a Full-Time Case Manager to provide persons over 60 years of age with comprehensive coordination of health services, financial assistance, and referrals through the LI Sound Senior Care Program in Greenport. Requirements

Associates Degree required.
Bachelor's Degree in Human Services, Social Work, Sociology or related field required.
At least five years’ experience assessing and linking adults to needed services and programs required.
Excellent interpersonal and verbal and written communication skills required.
Proficient computer skills, including Microsoft Office required.

Nice To Haves

Bilingual in Spanish preferred.

Responsibilities

The Case Manager will conduct intakes, assessments, service planning and follow up.
Provide access and help navigate systems and programs.
Obtain all proper consent forms to coordinate care.
Work with government and community agencies.
Ensure clients receive needed services, including provision of transportation to appointments.
The Case Manager will conduct program outreach.
All other duties as assigned.
